We are seeking a Mental Health and Substance Use Care Coordinator to join our South Mental Health & Substance Use team in Vancouver, BC.


Salary:


The estimated annual salary for this position is approximately $86,000 CAD ($41.42 - $55.91 per hour), commensurate with experience and qualifications.


Job Description:


As a Mental Health and Substance Use Care Coordinator, you will be responsible for providing quality, holistic nursing care for clients with moderate to severe mental health conditions and/or substance use disorder.


Key Responsibilities:



  • Provide evidence-informed nursing care to clients, effectively applying the nursing process to identify and mitigate risks to client health and promote safety and wellness through collaborative, client-centred, strengths-based, interdisciplinary care planning.
  • Develop and implement individualized care plans that address the unique needs of each client, taking into account their cultural background, preferences, and values.
  • Collaborate with an interdisciplinary team to provide comprehensive care to clients, including crisis intervention and management, psychopharmacology, and case management.
  • Communicate effectively with clients, families, and healthcare providers to ensure seamless care transitions and optimal outcomes.
  • Conduct thorough assessments of clients' physical, emotional, and psychological needs, identifying potential risks and developing strategies to mitigate them.
  • Monitor clients' progress, adjusting care plans as needed to ensure effective treatment outcomes.

Required Qualifications:


To be successful in this role, you must have:



  • A degree in Nursing or a related field.
  • Current practicing registration and authorization to practice as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years recent experience providing nursing care for individuals with mental health conditions and/or substance use disorders.
  • Valid driver's license, as local area travel may require the use of a vehicle.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
  • Demonstrated knowledge of mental health and substance use theory and practice, including therapeutic communication, crisis intervention and management, and psychopharmacology.
  • Ability to work independently and adapt to changing priorities and circumstances.
