In accordance with the Mission, Vision and Values of Provincial Health Services Authority, patient safety is a top priority and a responsibility shared by everyone at PHSA. As a result, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Key Responsibilities- Establish Therapeutic Relationships: Develop and maintain effective relationships with patients and families to provide emotional support and assess their needs.
- Assess and Identify Patient Needs: Conduct thorough assessments to identify patients' actual and potential strengths and limitations, including physiological, psychological, social-cultural, and spiritual needs.
- Develop and Implement Care Plans: Contribute to the development and modification of individualized care plans, identifying patient priorities and suggesting revisions as needed.
- Perform Nursing Procedures: Conduct various nursing procedures, such as taking blood pressures, heights, and weights, and communicate patient information to in-charge nurses, physicians, and other team members.
- Monitor Patient Status: Monitor patients' status in relation to anticipated outcomes, evaluate the effectiveness of nursing interventions, and report unanticipated changes and concerns to the nurse-in-charge.
- Maintain Accurate Records: Maintain complete and accurate computerized and paper records through methods such as tracking results, documentation, and inputting data in accordance with established procedures and policies.
- Medication Administration: Review medication administration records, check with the nurse-in-charge or physician regarding concerns, and store patient medications according to standards and agency directives.
- Deliver Medications: Deliver medications consistent with the written care plan, physician orders, and in accordance with the standards of British Columbia College of Nursing Professionals and established procedures.
- Assist with Examinations and Treatments: Assist RN/RPNs and physicians in the performance of various examinations, treatments, and diagnostic procedures, such as physical examinations.
- Collaborate with Treatment Team: Work in collaboration with the treatment team to assist patients with their program requirements, monitor program requirements, and use motivational interviewing techniques.
- Ensure Infection Control: Ensure that infection control protocols are followed by all patients and personnel, handle, store, and dispose of workplace hazardous materials and sharps in accordance with established procedures.
- Perform Security Functions: Perform regular security functions to maintain the safety and security of patients, staff, families, and the general public, and take immediate action to mitigate risks and report all real and potential risk issues.
- Graduation from a recognized program for Practical Nurses plus 1 year of experience working with offenders and/or in secure healthcare settings, or an equivalent combination of education, training, and experience.
- Current full practicing licensure with the British Columbia College of Nursing Professionals (BCCNP).
- Occupational First Aid Level 2 certification (OFA Level 3 certification for positions at the Ford Mountain Correctional Centre).
