Job Summary
We are seeking a Mental Health and Community Care Specialist to join our Intensive Care Management team in Sechelt, BC. As a Mental Health and Community Care Specialist with VCH, you will provide mental health services to individuals in need.
Key Responsibilities:
- Conduct comprehensive assessments of clients' mental health needs, develop treatment plans, and provide counseling and support to help them achieve their goals.
- Collaborate with interdisciplinary teams to coordinate client care and ensure continuity of services.
- Provide crisis intervention and support to clients in times of need.
- Work with community agencies and healthcare providers to ensure that clients receive the resources they need.
- Participate in case conferences and meetings with members of the multidisciplinary team.

Qualifications
To be successful as a Mental Health and Community Care Specialist, you will require:
- A diploma in nursing or a related field, along with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years of recent experience working with individuals with mental health issues or an equivalent combination of education and experience.
- Broad knowledge of crisis intervention and counseling skills, as well as mental health illness and treatment.
- Ability to communicate effectively with clients, families, and colleagues, both verbally and in writing.
