About the Opportunity

The Health Care Social Worker is a vital member of the interdisciplinary healthcare team, providing collaborative, holistic care across the primary to quaternary care continuum. This role focuses on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being. The social determinants of health are embedded in social work theory and practice, and the dynamic and interactive process of engagement, assessment, planning, intervention, and evaluation is a key aspect of this position.

About You

We are seeking a highly skilled and experienced Social Worker with a Bachelor's degree in Social Work from a recognized university. The ideal candidate will have current registration with the Nova Scotia College of Social Workers and a minimum of two years of post-BSW clinical experience. Knowledge and understanding of the Diagnostic & Statistical Manual of Mental Disorders are essential, as is previous experience working in adult community mental health. The successful candidate will demonstrate a client-/family-centered approach, familiarity with various evidence-based treatment approaches, and excellent communication skills. Competencies in other languages, particularly French, are an asset.

Key Responsibilities
  • Provide collaborative, holistic care to patients/clients across the primary to quaternary care continuum
  • Focus on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being
  • Embed social determinants of health in social work theory and practice
  • Engage in dynamic and interactive processes of engagement, assessment, planning, intervention, and evaluation
  • Work collaboratively with community partners to provide comprehensive care
