Social Worker

Fraser Health is seeking a skilled Social Worker to join our Acute Care team. As a Social Worker - Acute Care Specialist, you will provide therapeutic clinical counselling services to patients and families in the assigned clinical area within Acute Care.
Key Responsibilities:- Conduct social work assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Provide consultation to patients, families, and healthcare professionals on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical area.
- Develop, coordinate, and deliver in-services, presentations, and support groups to patients and families.
- Provide leadership to the interdisciplinary team in care planning for the effective utilization of patient resources.
- Make effective referrals to community resources and collaborate with community health and social agency personnel.
- Bachelor's Degree of Social Work from an accredited School of Social Work.
- One (1) year's recent, related experience in a hospital setting or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers.
