Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team in Acute Care. As a key member of our interdisciplinary team, you will provide therapeutic clinical counselling services to patients and families, conducting social work assessments and providing consultation to healthcare professionals.
Key Responsibilities- Provide direct social work services to patients and families, exploring emotional responses and facilitating change in coping styles, behaviors, and attitudes.
- Participate in interdisciplinary treatment teams to coordinate services, manage cases, and facilitate meetings.
- Conduct psychosocial assessments of patients and families, gathering social data and interpreting it using various modalities.
- Provide consultation to patients, families, and healthcare professionals on high-risk factors, recovery, and community practices.
- Maintain patient records, documenting assessments, charting patient information, and preparing progress notes and reports.
- Provide leadership in discharge planning, interviewing patients and families to assess and determine care plans.
- Arrange family meetings to develop and coordinate care plans and community services.
- Communicate with community health and social agencies, collaborating in assessments and referrals.
- Act as a resource to patients and families, providing information on community resources and access to these resources.
- Participate in local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery.
- Bachelor's Degree in Social Work from an accredited school.
- One year's recent experience in a hospital setting or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers.
