We are seeking a highly skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities- Assess patient care needs and develop individual care plans in collaboration with patients and their families
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, respecting their values and preferences
- Balance patient needs with available resources, ensuring timely and effective care delivery
- Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario)
- 2+ years of recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively
- Experience working with diverse patient groups, including multicultural, homeless, palliative, and geriatric populations
- Case management experience or recent related community experience
- Ability to speak French or another second language
