Job Summary

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at Extendicare Canada Inc. in a Long Term Care setting. As a Nurse Practitioner, you will be responsible for providing comprehensive health services to residents, including health assessments, diagnosis, and treatment plans.

Responsibilities
  • Deliver primary health care to residents in long term care, managing acute and chronic medical conditions, and promoting health and wellness.
  • Provide clinical leadership to the LTC team, ensuring resident care needs are met, and collaborating with other healthcare professionals to achieve optimal outcomes.
  • Conduct health assessments, using evidence-based tools and techniques, and develop person-centered plans of care.
  • Order diagnostic investigations, interpret results, and make informed decisions about resident care.
  • Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions.
  • Collaborate with Extendicare leaders and community health services to promote continuity of care and improve resident outcomes.
  • Participate in quality improvement initiatives, research, and education to enhance resident care and advance nursing practice.
Qualifications
  • College of Nurses of Ontario registration in the Extended Class.
  • Baccalaureate in Nursing (BScN), Master's Degree in Nursing (MScN or MN) an asset.
  • Membership with the Nurse Practitioners' Association of Ontario (NPAO) (preferred).
  • Has acquired professional liability insurance.
  • National certification in gerontology an asset.
