Patient Care Manager

Job Summary
Mackenzie Health is seeking a highly skilled and experienced Patient Care Manager to join our team at CVH 5 Mental Health. As a key member of our management team, you will be responsible for coordinating and directing the unit, as well as overseeing human and fiscal resources.
Key Responsibilities
- Collaborate with the Program Director, Medical Director, Educator, and other allied health members to ensure high-quality services and care.
- Plan, implement, and integrate plans that align with the Medicine Program and the organization's vision and strategic directions.
- Lead and manage a team of healthcare professionals to provide exceptional patient care and services.
- Develop and maintain effective relationships with stakeholders, including patients, families, and healthcare providers.
- Ensure compliance with regulatory requirements, policies, and procedures.
- Monitor and evaluate the performance of the unit, identifying areas for improvement and implementing changes as needed.
- Provide coaching, mentoring, and training to staff to enhance their skills and knowledge.
- Stay up-to-date with industry developments, best practices, and research to inform decision-making and improve patient outcomes.
Requirements
- Completion of a Baccalaureate Degree in Nursing or other regulated health professional field.
- Nurse Certificate of Competence from the College of Nurses of Ontario or another regulated Health Professional College.
- Minimum of 2-5 years clinical frontline leadership experience in mental health.
- Excellent interpersonal, communication, coaching, mentoring, and team building skills.
- Strong analytical, problem-solving, critical thinking, and decision-making skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Commitment to professional development and staying current with industry developments.
Preferred Qualifications
- Specialty certification an asset.
