Registered Nurse/Registered Psychiatric Nurse

Fraser Health is seeking a skilled Registered Nurse/Registered Psychiatric Nurse to join our team at Chenchenstway in Burnaby, BC. As a member of our interdisciplinary team, you will provide person-centered care to diverse populations, promoting resident/client choices and individualized care plans.
Responsibilities- Collaborate with the interdisciplinary team to identify current or potential care concerns and strengths, planning interventions and evaluating outcomes.
- Provide direct nursing care, including wound care, dementia care, and palliative care, in accordance with established policies and standards of nursing practice.
- Collect information from various sources, using skills of observation, communication, and physical assessment, to enable the provision of quality care.
- Document relevant observations, assessments, and interventions, including resident/client and family teaching and evaluation of care.
- Communicate resident/client assessments and changes to Physicians, Resident/Client Care Coordinator, and the interdisciplinary team.
- Act as an advocate to protect and promote residents'/client's rights to autonomy, respect, privacy, dignity, diversity, and access to information.
- Work in partnership with Licensed Practical Nurses and other interdisciplinary team members, sharing information, demonstrating, and/or providing guidance on clinical tasks.
- Participate in quality improvement activities, gathering information, providing input into proposed policies, and revising current policies.
- Maintain records, prepare reports, and collect statistical and quality improvement data for research and evaluation purposes.
- Maintain a safe working environment, following health and safety guidelines, ensuring equipment is in safe working condition, and reporting unsafe, hazardous conditions.
Education and Experience:
- Graduate of an approved School of Nursing or Psychiatric Nursing.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year's recent, related gerontological nursing experience or an equivalent combination of education, training, and experience.
Skills and Abilities:
- Knowledge of nursing theory and evidence-informed practice.
- Knowledge of health care disciplines and their roles in health care.
- Ability to apply clinical knowledge in practice.
- Ability to apply critical thinking skills, problem solve, and effectively resolve conflict situations.
- Ability to communicate effectively both verbally and in writing.
- Ability to provide leadership and clinical guidance.
- Ability to work independently and as a member of the interdisciplinary team.
- Ability to plan, organize, and prioritize work in a self-directed manner.
- Ability to operate related equipment.
- Physical ability to perform the duties of the position.
