We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health Authority. As a Gerontological Care Specialist, you will play a vital role in providing high-quality care to our patients and residents.
Key Responsibilities- Collaborate with the interdisciplinary team to identify care concerns and develop individualized care plans.
- Provide direct nursing care, including wound care, dementia care, and palliative care, in accordance with established policies and procedures.
- Collect and document relevant information to enable the provision of quality care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery.
- Participate in quality improvement activities and contribute to the development of policies and procedures.
- Provide leadership and clinical guidance to other healthcare professionals.
- Maintain accurate records and reports, and contribute to the collection of statistical and quality improvement data.
- Graduate of an approved School of Nursing or Psychiatric Nursing.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
- One (1) year's recent, related gerontological nursing experience or an equivalent combination of education, training, and experience.
- Current Level 1 First Aid certificate.
