Job Overview

Spectrum Health Care is a leading provider of home care services, committed to delivering high-quality patient care in the comfort of their own homes. We are seeking an experienced Home Support Supervisor to lead our team of Personal Support Workers and provide exceptional patient care.

Estimated Salary: $60,000 - $80,000 per year

Job Description

The Home Support Supervisor will be responsible for:

  • Leading a team of Personal Support Workers to deliver qualified personal care to patients in their homes;
  • Traveling to patients' homes in community and facility settings;
  • Working collaboratively with multidisciplinary teams in both community and facility settings;
  • Assessing patient needs, formulating care plans, and monitoring progress towards outcomes;
  • Managing staff, conducting performance appraisals, and implementing progressive disciplinary action;
  • Gathering, recording, and analyzing patient and employee feedback to maintain awareness of key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion rates;
  • Ensuring timely resolution of patient concerns and complaints;
  • Promoting Spectrum Health Care through participation in local healthcare committees and community events;
  • Participating in ongoing internal and external continuing education for professional development.
Qualifications

To be successful in this role, you will need:

  • A current registration with the Colleges of Nurses of Ontario in good standing;
  • A minimum of three (3) years' experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting;
  • Exceptional interpersonal skills and ability to handle difficult situations objectively;
  • Ability to write reports and communicate effectively in writing and verbally;
  • Excellent English verbal and written communication skills (additional language skills considered an asset);
  • Solid knowledge of the principles, practices, and methods of community nursing and service delivery to patients, as well as program development, implementation, and evaluation;
  • A valid driver's license and access to a reliable vehicle;
  • Basic proficiency in Microsoft Word, Excel, and ability to learn proprietary software.
