About the Role:- Provide direct psychosocial care, counseling, and group work to clients and their families.
- Develop and implement individualized care plans to meet the unique needs of each client.
- Collaborate with other healthcare providers and the multidisciplinary team to ensure comprehensive care.
- Provide education and linkage to relevant specialized complex care resources to support client self-management.
