We are seeking a highly skilled and dedicated Primary Health Care Nurse Practitioner to join our team at Horizon Health Network. As a key member of our healthcare team, you will be responsible for providing high-quality care to our patients, promoting health and wellness, and collaborating with other healthcare professionals to achieve optimal patient outcomes.
Key Responsibilities- Conduct comprehensive health assessments and develop individualized care plans to address the unique needs of each patient.
- Order and interpret diagnostic tests, prescribe medications, and provide evidence-based treatments to patients with acute and chronic conditions.
- Collaborate with other healthcare professionals to provide comprehensive care to patients, including referrals to specialists and community resources as needed.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Provide education and support to patients and their families to promote health and wellness, and empower them to take an active role in their care.
- Active registration as a nurse practitioner with the Nurses Association of New Brunswick.
- Minimum of 1 year of clinical practice experience as a NP within the last 5 years.
- Ability to work independently and in teams, with excellent communication and interpersonal skills.
- Demonstrated understanding of privacy and confidentiality, evidence-based nursing practice, and mentoring cultures.
