Job Summary

Fraser Health is seeking a skilled Registered Nurse to join our team in a Primary Care setting. As a key member of our inter-professional team, you will provide direct patient care to clients of the Urgent Primary Care Centre (UPCC) or Primary Care Clinic, delivering timely and longitudinal primary care to unattached and attached vulnerable clients.

Responsibilities
  1. Assess clients and collect information from various sources to make informed nursing diagnoses and determine the best course of action.
  2. Process physician or nurse practitioner orders, administer medications, and monitor patient responses.
  3. Establish a therapeutic relationship with clients, promoting autonomy and dignity in decision-making and care planning.
  4. Screen referrals, provide individualized client assessments, and develop inter-professional care plans to address complex client needs.
  5. Collaborate with primary care providers, clients, families, and other healthcare professionals to coordinate care and services.
  6. Develop and implement comprehensive shared client care plans, ensuring seamless transitions across the healthcare continuum.
  7. Provide direct planned and unplanned client care, identifying additional care services required to meet client needs.
  8. Review client care plans, monitor progress, and evaluate the effectiveness of care plans to achieve optimal client outcomes.
  9. Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based best practices.
Qualifications

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One year of recent related clinical experience in acute care, urgent/emergency care, or a related clinical area. Valid BC Driver's license and access to a personal vehicle for business-related purposes.

Skills and Abilities

  • Ability to complete initial and ongoing client assessments and provide nursing care through therapeutic interventions.
  • Effective communication and interpersonal skills.
  • Ability to integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
  • Knowledge of broad health care services, community resources, and their role in providing a continuum of care.
  • Ability to promote client-focused care, including sensitivity to diverse cultures and preferences.
  • Ability to work independently and as a member of an inter-professional team.
