Care Coordinator Role Overview

At Lifemark Health Group, we are seeking a highly organized and detail-oriented Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in fostering strong relationships between our clinic and its customers, including patients, staff, funders, and referral sources.

This position requires excellent communication skills, both written and verbal, as well as strong organizational and time management skills. The ideal candidate will be able to multi-task and prioritize tasks effectively while maintaining a high level of professionalism.

Key Responsibilities:

  • Foster positive relationships with patients, staff, and external customers through efficient administrative support and coordination of clinic activities.
  • Coordinate patient intake, data entry, answering phone calls, outbound calling, receiving faxes and emails, invoicing, and payments.
  • Assist patients with completing paperwork at the clinic to avoid errors and inaccurate information.
  • Coordinate scheduling of patient appointments and maintain accurate patient medical records.
  • Collect payment from patients at time of service.
  • Track coverage for all funding streams to ensure approval.
  • Complete data entry and billing tasks accurately and efficiently.
  • Provide administrative support to the Care Coordinator Manager and Clinic Director.

Qualifications & Core Competencies:

  • High School Diploma, G.E.D., or equivalent required.
  • Excellent customer service skills and telephone etiquette essential.
  • High degree of organizational skills and ability to prioritize and multi-task.
  • Excellent communication (verbal/written) and interpersonal skills necessary.
  • Strong computer and data entry skills required.
  • Flexible and adaptable to change.
  • Experience in a physiotherapy clinic an asset.

Salary Information:

The estimated annual salary for this position is $45,000 - $55,000, depending on experience.
