About the Role

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at Collage HR. As a key member of our Community Inclusion program, you will play a vital role in supporting adults with diverse abilities to reach their full potential and enhance their quality of life.

Key Responsibilities
  • Develop and implement individualized plans to support clients in achieving their goals and enhancing their independence and confidence.
  • Provide recreational activities and opportunities to help clients build relationships, social skills, and physical and emotional health.
  • Support clients with mobility challenges, including transporting them in agency vans and providing physical support as needed.
  • Monitor clients' personal care and health needs, and perform personal care tasks as required.
  • Collaborate with clients, caregivers, and other professionals to assess individual needs and develop and implement individualized plans.
  • Assess health and safety concerns and ensure protocols are in place to maximize health and safety standards.
  • Administer medication as needed, adhering to agency medication policies and procedures.
  • Ensure accurate and timely documentation of client progress and activities.
Requirements
  • Community or School Support Worker Certificate, with a Diploma an asset.
  • Minimum 1 year of related experience working with individuals with cognitive, physical, and other diverse abilities.
  • Or an equivalent combination of education, training, and experience.
  • Valid Class 5 Driver's license and safe driving record.
  • Physical requirements of the job include supporting individuals in wheelchairs, lifting, transferring, and assisting clients safely in and out of vehicles.
  • Following training is required and provided to the successful candidate free of charge, including CPR-C, Mandt, Safe Food Handling, Medication Management, Anti-Bullying and Harassment, and CLBC Privacy Consent.
  • Criminal Record Checks with vulnerable sector search required.
