About this Role
Vancouver Coastal Health is seeking a skilled Nurse Practitioner to join our Vancouver Community Palliative Care team.
Key Responsibilities
- Assess patients and develop comprehensive care plans to address their physical, emotional, and social needs.
- Collaborate with healthcare teams to provide high-quality patient care and support.
- Develop and implement population-based strategies to improve health outcomes and promote health equity.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Provide education and support to patients, families, and caregivers to promote self-care and independence.
Requirements
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives.
- Four years of recent, relevant clinical nursing experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE).
