Client Care Supervisor RN/RPN Position

CHATS is seeking a skilled Client Care Supervisor to provide client care needs assessment, case management, supervision, and clinical field support for in-home and community health care providers. This position has an emphasis on in-home services and includes community programs when assessment/case management is required. The successful candidate will also provide support in the teaching of special tasks and transfer of controlled acts by exception to Adult Day and Assisted Living Services.
This is a full-time position, 35 hours per week, with occasional work outside of business hours. The ideal candidate will have a strong background in nursing, excellent assessment, communication, and problem-solving skills, and experience working with seniors and/or provision of client services in the community.
The successful candidate will be responsible for conducting home visits, teaching and supervising delegated acts to Personal Support Workers, and providing direct services to clients including needs assessments, ongoing case management, and appropriate interventions in complex care situations to promote safe delivery of care. They will also foster key relationships with other related agencies and referral sources.
