Client Care Supervisor RN/RPN

Job Summary:
CHATS is seeking a highly skilled and compassionate Client Care Supervisor RN/RPN to join our team. As a Client Care Supervisor, you will be responsible for providing client care needs assessment, case management, supervision, and clinical field support for in-home and community health care providers.
Key Responsibilities:
- Conduct home visits and provide direct services to clients, including needs assessments, ongoing case management, and appropriate interventions in complex care situations.
- Supervise and mentor Personal Support Workers, teaching and delegating tasks as needed.
- Collaborate with the interdisciplinary team to ensure comprehensive care planning and coordination.
- Develop and maintain relationships with community agencies and referral sources to ensure seamless care transitions.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) with current certificate of registration with the College of Nurses of Ontario.
- Three to five years' experience in client care, preferably in a community setting.
- Excellent assessment, communication, and problem-solving skills.
- Demonstrated teaching, mentoring, and coaching skills.
- Supervisory experience preferred.
- Current CPR/first aid certification.
- Proficiency in MS Word, Excel, Outlook, and database applications (GoldCare).
- A valid Ontario driver's license and a vehicle are required for this position.
