Job Summary:

Baycrest is seeking a skilled and compassionate Program Assistant to join our team at the Terraces Memory Residence. As a key member of our care team, you will play a vital role in delivering exceptional care and support to our residents.

The ideal candidate will possess a strong background in gerontology, activation, and clinical care, with experience working with individuals living with dementia in residential settings. You will be responsible for delivering the overall care experience to residents and their families, embodying the model of care and its philosophy and values into daily practice.

Key Responsibilities:
  • Deliver exceptional care and support to residents, enabling choice and autonomy while supporting their physical and personal care needs.
  • Initiate and maintain relationships with residents, their families, and other care providers to enable trust, open communication, and a care community.
  • Support the delivery of mealtime experiences, including set up, food preparation, baking, and feeding.
  • Co-design and operationalize individualized care plans in partnership with residents, families, and other care providers.
  • Manage responsive behaviors through exploring and applying non-pharmacological interventions, including activities and strategies to meaningfully engage residents.
  • Prepare daily programming space and activities for individuals and groups based on their individual choices, interests, and daily goals.
Qualifications:
  • Current P.S.W. certificate from a recognized community college or educational institute or equivalent combination of education and experience required.
  • Registration in good standing with the Ontario Personal Support Worker Registry.
  • Completion of certificates in gerontology, activation, or related coursework preferred.
  • Ontario Secondary School Diploma or recognized equivalent required.
  • Grade 12 English equivalency required.
  • Certificate in U-First and/or Alzheimer Society for Dementia Care Training Program (DCTP) preferred.
  • Completion of Mental Health First Aid preferred.
  • Certificate in Gentle Persuasive Approach (GPA) preferred.
  • Certificate in Montessori Activities for Dementia preferred.
  • Food Handling Certification preferred.
  • Current Basic Cardiac Life Support (BCLS) required.
