Indigenous Patient Navigator

About the Role
We are seeking a highly skilled and compassionate Indigenous Patient Navigator to join our team at PHSA. As a key member of our healthcare team, you will play a vital role in providing culturally safe and competent care to Indigenous patients.
Key Responsibilities
- Provide consultative and navigational services to patients referred to PHSA programs and services, supporting cultural interpretation and/or health literacy translation services.
- Support patients in communications with physicians, nurses, and other frontline care staff, ensuring culturally safe and competent care.
- Coordinate access and coordination of services for patients within the designated region, including interagency, Indigenous communities, and other health programs and services.
- Provide client support services within the hospital care system/community, assessing patient needs and ability to access and receive culturally-safe care.
- Advocate for patient needs within the healthcare system, liaising with other community and regional health organizations to improve patient care.
- Support healthcare providers and staff members in providing culturally safe care, coordinating client/family spiritual care requests and/or access to sacred spaces.
- Maintain and update documentation and records related to patient needs, developing and maintaining a regional inventory of care-related programs and services.
- Support the development of regional strategies to improve care for Indigenous patients, engaging Indigenous communities and organizations.
- Provide a link between Indigenous communities and PHSA healthcare providers, supporting Indigenous peoples' understanding of the provincial healthcare system.
Requirements
- Diploma or degree in a related health discipline, health or social sciences, or patient care/clinical related discipline.
- 1-3 years' recent related experience working in Indigenous healthcare.
- Ability to travel, valid driver's license required.
- Demonstrated knowledge and experience working with Indigenous communities and organizations.
- Extensive knowledge of Indigenous peoples' demographics, languages, cultures, customs, and traditions.
- Experience or knowledge of the healthcare system and community-based resources within the region.
- Demonstrated knowledge, understanding, and application of cultural safety and competency within healthcare.
- Strong verbal and written communication skills.
- Ability to work effectively within a multi-disciplinary team.
