As a Nurse Coordinator with VHA, you will play a crucial role in supporting East Toronto Health Partners' (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program.

This short-term transitional program involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare, and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community.

You will be part of a multi-disciplinary team that works as one team, along with the client and designated family, to address their complex health and social needs in the development of a single care plan.

The key focus of this role is to provide and coordinate home health nursing services to a diverse group of individuals, families, and groups according to the College of Nurses of Ontario Practice Standards, the policies and procedures of VHA, the program where they provide services, and the Standards of Practice in Community Health Nursing.

Key areas of accountability include assessing, monitoring, and making care planning/care coordination, evaluating care plan interventions, ensuring quality of care, demonstrating professionalism, leadership, judgment, and accountability, supporting client safety care, maintaining documentation standards, and informing nursing supervisor of any client activities and relevant concerns or changes.

Required qualifications include being registered and in good standing with the College of Nurses of Ontario, membership of your Professional Association preferred, RPN diploma or RN diploma/degree from a recognized university or college, valid First Aid or CPR Certification, acute Care and Medical-Surgical nursing experience is an asset, knowledge of community nursing is an asset, demonstrate effective communication and documentation skills, reliable attendance, punctuality, and availability for the role, commitment to modeling lifelong learning and professional growth, critical thinking skills to provide client care, and a valid Ontario driver's license (G License) and a reliable vehicle for traveling.
