As a member of our team, you will have the freedom and flexibility to manage your own schedule, working with diverse clients in their homes, schools, retirement homes, and other community settings.

As a nursing specialist with VHA Home HealthCare, you will provide complex care and specialized nursing services to clients 18 years old and above, requiring support at home. You will work collaboratively with our interdisciplinary team to develop and implement personalized care plans, ensuring optimal outcomes for our clients.

Key Areas of Accountability
  • Assess, monitor, and make care planning/care coordination decisions that prioritize client safety and well-being.
  • Evaluate care plan interventions through ongoing reassessment and evaluation results, making adjustments as needed.
  • Ensure the quality of care by identifying and acting on factors that affect practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients, families, and healthcare teams.
  • Maintain accurate and timely documentation, adhering to standards set by the College of Nurses of Ontario.
Required Qualifications:
  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • Membership of your Professional Association preferred (RNAO/WeRPN).
  • RN diploma/degree or RPN diploma from a recognized university or college.
  • Hold a valid First Aid or CPR Certification.
  • A moderate level of wound care or venipuncture knowledge and experience is an asset.
  • Acute Care and Medical-Surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commitment to modeling lifelong learning and professional growth, applying critical thinking skills to provide exceptional client care.
  • A valid Ontario driver's license (G License) and a reliable vehicle for traveling are required for visiting roles.
