About the Role:
We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Sunny Hill Health Centre in Vancouver, BC.
Key Responsibilities:
- Assess and identify patients' and families' actual potential strengths and limitations using physiological, psychological, social-cultural and spiritual needs.
- Contribute to the development and modification of individualized nursing care plans in collaboration with the RN and input from the family.
- Deliver and collaborate in planning and coordinating the nursing care of patients, including documenting and updating information about the patient's condition.
- Review and interpret the established nursing plan of care, provide nursing care and assist patients with hygiene, grooming, toileting needs, ambulation and mobility, and feeding.
- Follow diagnostic treatment plans, including collecting specimens and preparing and caring for patients undergoing diagnostic tests, examinations, or procedures.
Requirements:
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
- Current full practicing licensure with the British Columbia College of Nurses and Midwives (BCCNM).
- Effective communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks.
- Knowledge of social, economic, political and historical realities impacting indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism.
