Mental Health Support Worker, Provincial Assessment Centre

About the Role
We are seeking a highly skilled and compassionate Mental Health Support Worker to join our team at the Provincial Assessment Centre. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals with complex mental health and substance use challenges.
Key Responsibilities
- Monitor and observe patients, documenting their progress and reporting any concerns or changes in their behavior or medical condition.
- Participate in the admission, case planning, and discharge process, working collaboratively with the interdisciplinary team to develop and implement individual care plans.
- Establish and maintain rapport with patients, providing emotional support and counseling, and assisting with problem-solving and daily living activities.
- Coordinate services with medical professionals and community resources, acting as an advocate for patients and providing information on available services.
- Plan and facilitate groups, such as sports and exercise, social support groups, and social activities, to support patient skill development and well-being.
- Prepare and maintain patient charts, entering information into the computerized client database and completing standard forms as required.
- Maintain a safe and clean environment for patients and staff, performing general housekeeping and cleaning duties as needed.
Requirements
- Diploma from a recognized Social Services Support Worker educational program, or equivalent combination of education and experience.
- Two years' recent, related experience, or an equivalent combination of education and experience.
- Valid driver's license, with local area travel requiring the use of a personal vehicle.
