About the Opportunity

This is a designated position that requires a professional approach to support the development and implementation of PHC-DFP programs, services, and initiatives throughout the zone.

The successful candidate will work collaboratively with Primary Health Care staff, family physicians, and primary care providers, as well as other health professionals and community groups, to maintain awareness of existing supports within the community and surrounding areas.

The primary responsibilities of the prideHealth navigator include working with individuals and groups to support members of the 2SLGBTQIA+ communities to connect with appropriate services within the health system and within the community.

The prideHealth Navigator will develop close working relationships and extensive collaboration with a multitude of community members, partners, colleagues, and portfolio partners, including Nova Scotia Health Authority (Public Health, Community Mental Health, etc.), IWK, Family Practice community, government programs and agencies, community-based and non-profit organizations, research/academic partners, and other stakeholders.

The prideHealth Navigator will also work collaboratively with team members and partners to develop, implement, and evaluate a community-based wellness navigation model that guides their work.

The incumbent is a member of an inter-professional team and collaborates with colleagues to determine goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities.

About the Opportunity Continued

The prideHealth Navigator's activities are guided by PHC-DFP priority areas, multiple stakeholders, and by community needs and, therefore, requires a professional approach.

The successful candidate will work according to the mission, vision, values, and policies of Nova Scotia Health Authority and PHC-DFP to support the development and implementation of PHC-DFP programs, services, and initiatives throughout the zone.

About You
  • We are looking for a candidate with a Bachelor's degree from an accredited institution in one of the following health care disciplines: Occupational Therapy, Social Work, Registered Nurse, Recreation Therapy.
  • Registration with respective professional regulatory association is required if applicable.
  • The successful candidate will have a minimum of 3 years of experience, including clinical practice related to discipline, working in Primary Health Care setting/community-based team/community setting, and minimum one year recent mental health experience.
  • Facilitation/education skills utilizing adult education, self-management supports, and motivational enhancement principles (both individual and group settings) are required.
  • Experience working with people living with a chronic condition(s) is essential.
  • Experience in partnership development, stakeholder collaboration, and community development is also required.
  • Understanding of population health, health promotion strategies, and behavior change principles is necessary.
  • In-depth knowledge of local health authority and community-based services is required.
  • The successful candidate will demonstrate person-centered/family-focused, diversity-relevant approaches to care, as well as well-developed interpersonal, teamwork, oral, and written communication skills.
  • Demonstrates flexibility, adaptability, and willingness to change.
  • Demonstrates innovation in program planning, implementation, and evaluation, and quality improvement.
  • Computer skills (MS Office, email, and internet) are required.
  • Annual CPR-AED certification and annual WHMIS training and certification are necessary.
  • A valid driver's license and daily access to a reliable car for travel for work are required.
  • Competencies in other languages are preferred; French is an asset.
Hours
  • Permanent Full-time / 75 Hours Bi-weekly
  • Shifts may include evenings, weekends, and holidays.
  • Travel to various sites may be required.
