Job Summary

HAMILTON HEALTH SCIENCES is seeking a skilled Social Worker to join our team in the Emergency Department. As a member of our healthcare team, you will provide direct patient care through psycho-social screening and assessments, individual and family counseling, crisis intervention, bereavement counseling, liaison and referral to community agencies, advocacy, and discharge planning.

Responsibilities

  • Provide psycho-social screening and assessments to patients and families
  • Offer individual and family counseling to address mental health concerns
  • Develop and implement crisis intervention plans to support patients in crisis
  • Liaise with community agencies to provide referrals and support
  • Advocate for patients and families to ensure their needs are met
  • Develop and implement discharge plans to support patients in their transition to community care

Requirements

  • Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
  • Masters degree in Social Work from an accredited School of Social Work
  • Recent Emergency Department experience as a Social Worker (within 2 years) required
  • Previous crisis intervention experience
  • Demonstrated skills in the performance of a therapeutic role, including strong assessment, counseling, case management, and advocacy skills
  • Expert knowledge of community resources
  • Strong interpersonal, teamwork, and communication skills
