Pediatric Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our multidisciplinary team at Hamilton Health Sciences. As a Pediatric Social Worker, you will play a vital role in providing psychosocial assessments, developing and implementing intervention plans, and advocating for children and families in our ambulatory tertiary care setting.
Key Responsibilities- Conduct in-depth psychosocial assessments to identify child and family strengths, stressors, and needs
- Develop and implement intervention plans to address education, coping, and compliance
- Provide individual and family therapy, group work, consultation, case management, crisis intervention, and patient and family education
- Identify and make referrals to appropriate supports and resources, and assist families in accessing financial assistance programs when necessary
- Master's Degree in Social Work from an accredited School of Social Work
- Current certificate of Registration with the Ontario College of Social Workers and Social Service Worker
- Minimum three (3) years recent experience with children and families in an ambulatory tertiary care pediatric setting providing individual and family therapy
- Familiarity with the challenges facing children and families managing complex conditions
- Demonstrated expertise in assessment, psychoeducation, crisis intervention, and advocacy skills in working with children, teens, and their families
- Knowledge of community resources and referral process
- Familiarity with the Mental Health Act, Consent to Treatment Act, Child and Family Services Act
- Demonstrated ability to be self-directed, and work in a fast-paced, changing environment
- Excellent communication and teamwork skills
- Demonstrated commitment to ongoing learning and the ability to teach
- Proven ability to organize, prioritize, and problem-solve effectively
