Position Overview

The role is pivotal in facilitating the management of patient flow and optimizing resource utilization within the designated Service Delivery Organization, as well as across various services and locations as necessary. This position also serves as the primary contact for Care Coordination at the site level, focusing on system flow management and performance enhancement.

Main Responsibilities

  • Contributes to the design, planning, and execution of regional and provincial strategies aimed at enhancing patient flow.
  • Develops proficiency in Patient Flow Software and Bed Management systems.
  • Ensures a comprehensive approach to education and training regarding the use of Patient Flow Software and Bed Management tools.
  • Implements a regional quality assurance program related to the utilization of Patient Flow Software and Bed Management applications.
  • Engages in the planning, execution, delivery, and assessment of projects and initiatives aimed at meeting operational patient flow performance objectives.
  • Maintains bed access within facilities in accordance with established regional protocols.

Experience Requirements

  • A minimum of 5-10 years of recent experience in an acute medical, surgical, or critical care/emergency setting is required.
  • Experience in an operational capacity, particularly with an emphasis on transition planning and practices, is preferred.
  • Familiarity with computerized applications, including Microsoft Office and patient flow software, is advantageous.

Educational Qualifications

  • A baccalaureate degree in a relevant healthcare discipline is required.
  • A master's degree is preferred.
  • Education in Lean Six Sigma or a comparable qualification is preferred.

Certification and Licensure

  • Current registration with a professional association or licensing body is mandatory.

Skills and Competencies

  • Exceptional communication and interpersonal abilities.
  • Capable of working independently as well as collaboratively within a team.
  • Proven capacity to operate effectively within a project team environment.
  • Strong understanding of collaborative practice and inter-professional education and practice principles.
  • Demonstrated analytical and problem-solving skills.
  • Ability to efficiently utilize computerized clinical applications.

Physical Requirements

  • Willingness to work across all Winnipeg Health Region locations (acute/community/long-term care) and arrange personal transportation.
