Job Summary:

As a Care Coordinator, you will be responsible for providing exceptional customer service to patients, staff, and external customers, while ensuring efficient administrative support and coordination of clinic activities.

Key Responsibilities:

  • Answer telephone calls and greet patients and visitors with a friendly and professional demeanor.
  • Coordinate patient intake, data entry, and scheduling of appointments.
  • Assist patients with completing paperwork and ensure accurate information.
  • Collect payment at time of service and track coverage for funding streams.
  • Provide administrative support to the Care Coordinator Manager and Clinic Director.

Qualifications & Core Competencies:

  • High School Diploma, G.E.D. or equivalent.
  • Excellent customer service skills and telephone etiquette.
  • High degree of organizational skills and ability to prioritize and multi-task.
  • Excellent communication (verbal/written) and interpersonal skills.
  • Strong computer and data entry skills.
  • Flexible and adaptable to change.
