Job Title: Social Worker, Primary Care Specialist

Interior Health Authority is seeking a highly skilled Social Worker to join their Primary Care team in Oliver. This permanent full-time opportunity will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.

Key Responsibilities:
  • Provide direct social work services and care coordination to individuals, families, and groups.
  • Collaborate with primary care providers and the interdisciplinary team to provide psychosocial care, counseling, and group work.
  • Develop and maintain social work services to maintain or improve the health and social well-being of the identified population.
  • Act as a resource to patients and their families by providing information on community resources and access to these resources.
Requirements:
  • Bachelor's degree in Social Work or a related field.
  • Experience working in a primary care setting or with Indigenous communities.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
