### Job Summary

We are seeking a skilled Social Worker to join our team at the South Okanagan General Hospital in Oliver, B.C. As a key member of our interdisciplinary team, you will provide comprehensive assessment, care planning, and treatment coordination for designated clients and their families.

### Key Responsibilities

* Provide direct psychosocial care, counseling, and group work to clients and their families
* Work collaboratively with other healthcare providers to develop and implement care plans
* Facilitate client self-management and provide education on relevant health and social issues
* Develop and maintain social work services to improve the health and social well-being of clients

### Qualifications

* Bachelor's Degree in Social Work from an accredited university
* Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders
* Current full registration with the British Columbia College of Social Workers
* Current valid BC driver's license
