Physiotherapist - Home and Community Care Support Services

We are seeking a highly skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing patient-centered care to our clients in the comfort of their own homes.
Job Summary:The successful candidate will be responsible for assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners. You will work collaboratively with our multidisciplinary team to deliver high-quality care and support to our clients.
Key Responsibilities:- Assess and determine patient care needs and eligibility
- Provide access and referrals to community services
- Engage with patients, caregivers, and other healthcare practitioners
- Develop and implement treatment plans to meet patient needs
- Collaborate with the multidisciplinary team to deliver high-quality care and support
- Master's degree in Physiotherapy or equivalent
- Current certificate of registration with the College of Physiotherapists of Ontario
- Related professional experience in a Physiotherapy role
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
