We are seeking a highly skilled Physiotherapist to join our team at Home and Community Care Support Services. As a Physiotherapist, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Job Summary:The Physiotherapist will be responsible for assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners. The ideal candidate will have a strong background in physiotherapy, excellent communication skills, and the ability to work effectively in a team environment.
Key Responsibilities:- Assess and determine patient care needs and eligibility
- Provide access and referrals to community services
- Engage with patients, caregivers, and other healthcare practitioners
- Develop and implement treatment plans to meet patient needs
- Collaborate with other healthcare professionals to ensure comprehensive patient care
- Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
- Related professional experience in a Physiotherapy role
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
- Fluency in English and French is required
