Job Summary
We are seeking a highly skilled and experienced Forensic Community and Liaison Social Worker to join our team at PHSA. As a key member of our clinical services team, you will be responsible for providing high-quality social work services to individuals with complex mental health and substance use challenges.
Key Responsibilities
- Conduct comprehensive biopsychosocial assessments to identify client needs and develop individualized treatment plans.
- Provide community case coordination and advocacy services to support clients in accessing community resources and services.
- Collaborate with multidisciplinary teams to provide clinical services, including assessment, treatment, and support.
- Develop and maintain relationships with community partners and stakeholders to ensure seamless service delivery.
- Provide education and support to clients and their families on mental health and substance use issues.
- Participate in quality improvement initiatives to enhance service delivery and client outcomes.
Requirements
- Master's degree in Social Work from an accredited school of social work.
- Minimum 2 years of experience working in a mental health or social services setting.
- Registration with the British Columbia College of Social Workers (BCCSW) is required.
- Valid BC driver's license and access to a reliable vehicle.
- Current CPR certification.
Preferred Qualifications
- Experience working with Indigenous populations and knowledge of Indigenous cultural safety principles.
- Knowledge of the mental health and substance use system in British Columbia.
- Experience with case management and coordination of services.
