About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Health Care Social Worker to join our interdisciplinary healthcare team. As a key member of our team, you will provide collaborative, holistic care on the primary to quaternary care continuum.

Key Responsibilities
  • Provide psychosocial, emotional, and spiritual support to patients and families
  • Assess and address the social determinants of health through engagement, assessment, planning, intervention, and evaluation
  • Build on the strengths of patients and families by focusing on existing coping strategies and problem-solving skills
  • Develop and implement individualized care plans to meet the unique needs of patients and families
  • Collaborate with healthcare professionals to provide comprehensive care
  • Provide education and support to patients and families on navigating local community resources and agencies
Requirements
  • Bachelor's degree in Social Work from a recognized university
  • Current registration with the Nova Scotia College of Social Workers
  • 2 years' post BSW related clinical experience required
  • Previous palliative care experience preferred
  • Grief & Bereavement Training preferred
  • Previous experience working in community-based settings preferred
  • Demonstrated ability to use a client-/family-centered approach
  • Ability to complete biopsychosocial assessments
  • Excellent communication skills
  • Demonstrated ability to assist patients and families with navigation of local community resources and agencies
  • Travel to other sites may be required and must have access to a reliable vehicle
  • Competencies in other languages an asset, French preferred
