PSW Coordinator Job Description

At AgeCare Ltd., we are seeking a highly skilled and dedicated PSW Coordinator to join our team. As a key member of our care team, you will play a vital role in ensuring the highest level of care and support for our residents.

Key Responsibilities:
  • Collaborate with the Director of Care and senior management team to plan, organize, and direct resident personal care services.
  • Monitor and evaluate the quality of care provided to residents through direct supervision and audits.
  • Lead quality initiatives and participate in the CQI committee to promote excellence in care.
  • Develop and implement person-centered care plans and programs.
  • Assist the Nursing Team in the development and evaluation of personal care programs.
  • Liaise with local schools and instructors for student placement and coordinate the Student Placement Program.
Requirements:
  • Graduate of a Personal Support Worker Certificate/Diploma Program.
  • Experience working as a Personal Support Worker in Long Term Care.
  • Knowledge of procedures and techniques involved in administering simple treatments and related bedside care.
  • Behavioural Support Ontario (BSO) certification and Imagine training is an asset.
  • Strong computer literacy and typing skills.
