The Opportunity

The PSW Coordinator reports to the Director of Care and works collaboratively with the senior management team to plan, organize, direct, and coordinate resident personal care services within approved policies and procedures. The successful candidate will promote and support Personal Support Workers (PSWs) to provide the best care possible in accordance with AgeCare's mission and values.

Key Responsibilities
  1. Assumes full responsibility for the general day-to-day operations of the Personal Support Workers (PSWs).
  2. Participates in the hiring, orientation, evaluation, and corrective action of PSWs.
  3. Monitors the care and services provided to residents through direct supervision and formal and informal audits, on all shifts.
  4. Promotes improved practices through mentorship, coaching, and education.
  5. Ensures a high focus on quality within the home as a member of the CQI committee, assists mandatory program leads with quality initiatives.
  6. Leads in person-centred philosophy, Imagine initiatives, and restorative care approaches.
  7. Ensures all PSWs complete mandatory annual education.
  8. As a Lift Champion, responsible for leading the Lift & Transfer Program ensuring training and competencies of employees and students.
  9. Assists the Nursing Team in the development and evaluation of personal care programs.
  10. Leads monthly PSW & Imagine meetings.
  11. Attends resident care conferences as required.
  12. Liaison with local schools and instructors for student placement.
  13. Leads and coordinates the Student Placement Program for the home, which includes coordination of preceptors, orientation and onboarding, regular touchpoints with students and preceptors, and evaluation of placements.
Requirements
  1. Graduate of Personal Support Worker (PSW) Certificate/Diploma Program that meets the standards outlined in the Fixing Long Term Care Homes Act.
  2. Must have worked as a Personal Support Worker in Long Term Care.
  3. Knowledge of procedures and techniques involved in administering simple treatments and related bedside care to residents.
  4. Behavioural Support Ontario (BSO) certification and Imagine training is considered an asset.
  5. Strong computer literacy and typing skills; familiarity with programs such as Word, Excel, PCC, PPOC, Workday, UKG, and Outlook is an asset.
  6. Must understand the basics of good sanitation and clean techniques; knowledgeable with respect to the needs of geriatric residents.
  7. Must be mature with effective communication (both written and verbal) and interpersonal skills.
