Job Summary

We are seeking an experienced nursing leader to join our team as a Team Leader in Home and Community Care. Key Responsibilities
  • Coordinate Home Support Services, including supervising Community Health Workers and overseeing the implementation of client care plans.
  • Provide training and evaluation of personal care techniques, ensuring high-quality care for clients.
  • Collaborate with the multidisciplinary team to perform assessments, plan, implement, and provide personal and nursing care to clients in a community health setting.
  • Ensure that nursing procedures are carried out in accordance with the LPN Standards of Practice as outlined by BC College of Nurses and Midwives (BCCNM).
Requirements
  • Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
  • Two (2) years of recent, related experience, including one (1) year in a supervisory role, or an equivalent combination of training and experience.
  • Current valid BC Driver's License.
Preferred Qualifications
  • Ability to supervise others.
  • Demonstrated knowledge and skills within a specific clinical specialty or patient population.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to establish and maintain rapport with clients.
  • Ability to work independently and collaboratively with others.
