We are seeking an experienced nursing leader to join our team as a Team Leader in Home and Community Care. As a key member of our multidisciplinary team, you will be responsible for coordinating Home Support Services, supervising Community Health Workers, and providing training and evaluation of personal care techniques.
Key Responsibilities- Coordinate Home Support Services, including supervising Community Health Workers and overseeing the implementation of client care plans.
- Provide training and evaluation of personal care techniques, ensuring high-quality care for clients.
- Collaborate with the multidisciplinary team to perform assessments, plan, implement, and provide personal and nursing care to clients in a community health setting.
- Ensure that nursing procedures are carried out in accordance with the LPN Standards of Practice as outlined by BC College of Nurses and Midwives (BCCNM).
- Registration with BC College of Nurses and Midwives as a practicing LPN registrant.
- Two (2) years of recent, related experience, including one (1) year in a supervisory role, or an equivalent combination of training and experience.
- Current valid BC Driver's License.
- Ability to supervise others.
- Demonstrated knowledge and skills within a specific clinical specialty or patient population.
- Ability to communicate effectively, both verbally and in writing.
- Ability to establish and maintain rapport with clients.
- Ability to work independently and collaboratively with others.
