We are seeking a Licensed Practical Nurse (LPN) with supervisory expertise to lead the Home Support team. This position plays a crucial role in enhancing the quality of care provided to clients.
Key Responsibilities:
Collaborate with clients and their families to set achievable and safe care objectives, while assessing, planning, coordinating, and implementing services in an interprofessional team framework. Evaluate client eligibility for services, develop and adapt care plans, deliver direct care, and manage care to empower clients in navigating community resources.About the Community

Required Qualifications:
Education & Experience
Completion of a recognized Licensed Practical Nurse program. Active registration as a Licensed Practical Nurse with the relevant regulatory body. A minimum of two years of recent experience working with elderly clients and those with chronic or acute conditions. At least one year of experience in a supervisory capacity. Valid driver's license is required for local travel.Skills & Competencies:
Proven skills in medication administration as per the LPN Scope of Practice. In-depth understanding of various healthcare disciplines and their roles in client care. Familiarity with the standards set by the regulatory body for LPNs. Extensive knowledge of care for Medical, Surgical, and Palliative clients. Ability to lead and mentor team members, fostering a collaborative environment. Strong problem-solving skills and resourcefulness. Effective communication skills, both verbal and written, tailored to diverse audiences. Capability to work independently and in collaboration with others. Ability to manage multiple priorities under pressure. Proficiency in CPR techniques and computer literacy for client management systems. Physical capability to fulfill the responsibilities of the position.Additional Information:
