We are seeking a highly skilled Community Support Worker to join our team at Cochrane Temiskaming Resource Centre in New Liskeard. As a Community Support Worker, you will play a vital role in providing support to individuals with Developmental Disabilities, their families, and service providers.
Key Responsibilities- Act as the initial point of contact for individuals requesting services, representing their needs and completing intake assessments.
- Collaborate with the Professional Resource Team to determine service delivery and assist individuals and families in accessing community supports and government services.
- Provide support and guidance to individuals and families, promoting independence and self-advocacy.
- Develop and maintain relationships with community partners and service providers to ensure seamless service delivery.
- A Social Service Worker Diploma or Social Worker Degree, or proof of registration with the Ontario College of Social Workers and Social Service Workers.
- A good understanding of social work code of ethics and principles, and adherence to Ministry and CTRC policies and procedures.
- Proven ability to work in a diverse environment, with a strong focus on customer service and teamwork.
- Two years' experience in the human services field, with a preference for experience with various software programs.
- A valid Class G driver's license and use of a vehicle.
- Double COVID-19 vaccination.
- A competitive hourly wage of $29.28 - $31.63.
