Care Coordinator

About the RoleWe are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a key role in assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.
Key Responsibilities- Assess patient care needs and develop individual care and service plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families
- Balance patient needs and choices with available resources
- Membership in good standing with the College of Nurses of Ontario
- Recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
