About the RoleWe are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.
Key Responsibilities- Assess patient care needs and develop individual care and service plans in collaboration with patients and their families.
- Link patients with service providers and coordinate care plan delivery.
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources.
- Work collaboratively with healthcare teams to ensure seamless care delivery.
- Membership in good standing with the College of Nurses of Ontario.
- Recent experience in community health or a related field.
- Knowledge of the healthcare delivery system and community resources.
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
- Effective time management, prioritization, and organizational skills.
