Job Summary:

We are seeking a skilled Health Care Professional to join our team at WRHA Community Health Services. As a key member of our clinical team, you will provide high-quality patient care and support to our clients.

Key Responsibilities:

  • Provide direct patient care and support to clients with various health needs.
  • Work collaboratively with healthcare professionals to develop and implement patient care plans.
  • Conduct routine assessments and evaluations to identify patient needs and develop strategies for care.
  • Administer medications and treatments as prescribed by healthcare professionals.
  • Document patient information and progress in a timely and accurate manner.

Requirements:

  • Post-secondary education in a related field (e.g. nursing, healthcare administration).
  • Experience working in a healthcare setting, preferably in a clinical role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Physical stamina to perform job duties, including standing, walking, and lifting.
