About the Role

We are seeking a highly skilled Social Worker to join our team at Markham Stouffville Hospital Corporation. As a Social Worker, you will play a critical role in providing patient-centered care and support to individuals and families in our community.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop personalized care plans.
  • Provide short-term, goal-oriented support and counseling to patients and families dealing with grief, chronic life-altering issues, and end-of-life concerns.
  • Collaborate with the healthcare team, patients, and families to develop and implement patient care plans.
  • Cross-cover for other Social Workers within the hospital as needed.
  • Consult with community healthcare professionals to ensure seamless care coordination.
  • Maintain up-to-date knowledge of community resources and services.
  • Attend and participate in committees and task forces as required.
Requirements
  • Master of Social Work degree from an accredited institution.
  • Registration with the Ontario College of Certified Social Workers and Social Services Workers.
  • Recent related experience in a hospital environment.
  • Understanding of loss/adjustment and the grief process.
  • Demonstrated strengths in interpersonal relations, problem-solving, and conflict resolution.
  • Excellent communication and documentation skills.
  • Commitment to professional development and ongoing education.
  • Experience working with electronic documentation systems, preferably Meditech.
  • Good attendance and performance records, with the ability to maintain these standards.
  • Commitment to a healthy and safe workplace for self and others.
Work Schedule

The successful candidate will be available to work between Monday and Sunday in 8-hour day shifts, primarily between 08:30 and 16:30. The schedule is subject to change based on operational needs.

Compensation

The salary for this position is $57.08 per hour, as per the OPCOM13 grid.
