Job Title: Social Worker

Job Summary:

We are seeking a highly skilled and compassionate Social Worker to join our team at Bayshore Home Health. As a Social Worker, you will play a vital role in providing community-based therapy services to our clients, working collaboratively with healthcare professionals to promote optimal independence and improve quality of life.

Key Responsibilities:

  • Assess client needs and develop personalized care plans to address social, emotional, and cognitive functioning.
  • Communicate effectively with clients, families, caregivers, and healthcare providers to coordinate services and ensure seamless care transitions.
  • Manage a caseload efficiently, prioritizing tasks and meeting deadlines to ensure timely interventions.
  • Utilize a client-centered approach to identify, prioritize, and monitor potential psychosocial problems and goals.
  • Establish, implement, evaluate, and adjust psychosocial treatment plans in collaboration with clients, families, caregivers, and healthcare professionals.
  • Enhance or teach coping skills, relaxation techniques, stress management, anger management, and healthy grieving strategies.
  • Advise clients and caregivers on community resources and contact agencies on their behalf when necessary.
  • Determine when further social work intervention is no longer necessary and assist clients in planning for discharge.
  • Recommend and facilitate referrals to support services within the community.
  • Comply with continuous quality improvement initiatives and adhere to Bayshore Home Health policies and procedures.

Requirements:

  • Master's or Bachelor's degree in Social Work.
  • Active membership in good standing with the Ontario College of Certified Social Workers and Social Service Workers.
  • Professional liability insurance ($5 million minimum).
  • Police clearance that meets Bayshore Home Health policy requirements.
  • General computer skills and access to a computer with password protection and internet capabilities.
  • Minimum of two years of community-based therapy experience (preferred).
  • Valid driver's license and access to a vehicle in good operating condition (preferred).
  • Current CPR certification (Basic Rescuer).
