Key Responsibilities- Client Care and Support
- Monitor clients in accordance with treatment objectives and document observations in accordance with standard procedures.
- Provide emotional support and supportive counselling to clients, assist with problem-solving, and promote healthy coping mechanisms.
- Establish and maintain rapport with clients, respecting their individual needs and boundaries.
- Collaboration and Communication
- Participate in the admission, case planning, and discharge process for clients, collaborating with interdisciplinary teams to develop and implement individual care plans.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
- Administrative and Organizational Tasks
- Prepare and maintain accurate and comprehensive client charts, including updating records and completing standard forms as required.
- Enter information into computerized client databases, maintaining statistical information and forwarding designated forms/files to applicable areas.
- Teamwork and Professional Development
- Collaborate with colleagues to maintain a safe and clean environment for clients and staff, performing general housekeeping and cleaning duties as required.
- Participate in ongoing professional development, staying up-to-date with best practices and industry developments to enhance skills and knowledge.
- Education and Experience
- Diploma from a recognized Social Services Support Worker educational program, or equivalent combination of education and experience.
- Two (2) years' recent, related experience in a mental health or social services setting.
- Skills and Knowledge
- Knowledge of the principles of recovery in mental illness, community resources, and social services.
- Ability to establish and maintain rapport with clients, observe and recognize changes in clients, and deal effectively with conflict situations.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada, as per TRC and BC's Declaration on the Rights of Indigenous Peoples Act.
