We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our team as a Community Mental Health Specialist. This is an exciting opportunity to work with individuals with mental health and substance use issues, providing treatment and care management services in a community-based setting.
Key Responsibilities- Client Care and Assessment
- Conduct comprehensive mental health and substance use assessments to identify client needs and develop individualized care plans.
- Provide treatment and care management services to clients, including crisis intervention, individual counseling, and education.
- Collaboration and Communication
- Work collaboratively with the interprofessional care team, including physicians, primary care providers, and other healthcare professionals, to ensure comprehensive care.
- Communicate effectively with clients, families, and caregivers to ensure understanding and engagement in treatment plans.
- Documentation and Reporting
- Document client assessments, care plans, and treatment outcomes in accordance with established standards and procedures.
- Prepare and submit reports as required, including client progress and treatment outcomes.
- Professional Development
- Participate in ongoing education and training to maintain current knowledge and skills in mental health and substance use treatment.
- Engage in quality improvement activities to enhance client care and services.
- Education and Experience
- Graduation from an approved School of Nursing or Psychiatric Nursing.
- Two years of recent experience providing treatment services to individuals with mental health and/or substance use issues.
- Skills and Abilities
- Ability to assess and provide treatment to individuals with mental health and substance use issues.
- Knowledge of mental health and substance use treatment principles and practices.
- Effective communication and interpersonal skills.
- Ability to work collaboratively as part of an interprofessional care team.
