Job Summary

We are seeking passionate and caring Acute Care Community Nurses to join our team. As a member of our team, you will use your clinical skills to ensure that our clients receive the highest quality care, delivered with compassion, excellence, and reliability.

Responsibilities

  • Provide high-quality care to clients in their homes, using your clinical skills and expertise.
  • Work collaboratively with our interdisciplinary team to develop and implement care plans that meet the unique needs of each client.
  • Conduct thorough assessments of clients' physical and emotional needs, and develop care plans that address these needs.
  • Provide education and support to clients and their families, to promote independence and self-care.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal client outcomes.

Requirements

  • Current registration with the provincial licensing body.
  • Valid driver's license and reliable vehicle.
  • Graduation from an accredited School of Nursing.
